2023 ServSafe® EXAM ONLY - Ohio Manager Food Protection 

   Offered by Ohio State University Extension, Montgomery County in partnership with TAP Series

  • Meets Ohio Department of Health requirements for Certification in Ohio Manager Food Protection
  • The ServSafe® exam is proctored by Registered ServSafe® Proctor: Pat Holmes
  • There is an additional fee of $25 to reschedule an exam date missed without notification

The registration fee for Exam only is $75 non-refundable/non-transferable. Failed exams are subject to a $50 re-test fee. To avoid a rescheduling fee, call or email Pat before the exam starts

To pay by check: Make check to OSU Extension Mail: Attn Pat, OSU Extension, 580 Calumet Lane, Dayton, OH 45417.

EXAM LOCATION: at OSU Extension Office Meet me in the Lobby of Calumet Center), 580 Calumet Lane, Dayton, OH 45417 west of Rt 49 connector and ½ mile south of Drexel. Calumet Lane is just east of the Marathon Station.

EXAM DATE/TIME 2023. No entry once the exam starts. All exams start at 1 p.m. so arrive on time. If a scheduled exam is full, another date/time will be added. *** No exam scheduled in July.

SPECIAL ACCOMMODATION requests must be received 10 days before the exam date. ____________________

  • Bring: a copy of online Course Completion Certificate for my files, photo I.D., 3 pencils & eraser, mask with you. The exam will be proctored at Calumet Center by OSU Extension staff.  
  • Student takes the ServSafe® exam on their pre-selected date.
  • Exam answer sheets will be sent to the National Restaurant Association to be scored.
  • When the student passes the exam, a ServSafe® certificate can be printed at 

Scores are usually available within 5 -14 business days.

  • OSU Extension then will submit scores to the Ohio Department of Health requesting a Certificate of Ohio Manager Food Protection Certification and then email the Certificate to you.
